Doctors Care - Provides Certified Health Coverage Guidance for your Patients
Attention Practice Managers and Staff:
You may be familiar with Doctors Care and the services it has provided to the un/under-insured at its clinic located at 609 W. Littleton Blvd., over the years but did you know Doctors Care is also a certified assistance site for Connect for Health Colorado and Medicaid?
Doctors Care has trained Heath Coverage Guides on staff who can help your patients determine if they are eligible for subsidized private insurance plans or Medicaid, as well as help navigate through the questions your patients may have about these coverages.
The Health Coverage Guides at Doctors Care can help patients select plans that allow them to keep their current providers, ensuring that your patients stay at your practice. These services are provided at no cost to your patients.
So please note that if an uninsured individual or family visits your practice, you have a resource to help them. They may complete the following form so a Certified Health Coverage Guide can contact them for follow up:
http://www.doctorscare.org/_docs/Coverage%20Fax%20Back.pdf. This form can be integrated into your intake paperwork and completed forms may be faxed or emailed to Doctors Care.
Doctors Care will contact the individual(s) to arrange for an appointment.
If you have any questions about this service or would like a representative from Doctors Care to visit your practice and provide you a stack of forms, please contact Sarah Thoemke directly at firstname.lastname@example.org or 303-249-8821.
